All patients must go through screening protocols:

  1. Screened for COVID symptoms such as fever, cough, shortness of breath, chills, muscle aches, new loss of taste or smell.

  2. All patients and visitors must wear a mask. Visitors are not allowed, unless needed for care-giving services like an interpreter or parent with a child.

  3. Prior to your appointment, all patients need to fill out the online intake form:

What our staff members are doing to help keep you safe:

  1. Employees are screened daily for COVID-19 symptoms. All employees must wear a mask.

  2. Sanitize all exam rooms and equipment after every patient.

  3. All frames are disinfected by UV-C sanitizer.

  4. Disinfect all high touch areas, and clean our facility throughout the day.

  5. Wash hands / use hand sanitizer routinely.
